The audit separates what screenshots can prove from what requires code or assistive-technology testing. Semantics, screen-reader behavior, and performance are marked unverified when the available evidence cannot support a conclusion.

Provide the product, user task, start point, endpoint, audit boundary, and either a testable page or current screenshots in sequence. Add target devices, audience, known concerns, and desired depth. State whether you also need a design board or remediation checklist.
